Detail

Add a flag to force user to use OpenID for login if they have in the past.

If this flag is set: {OpenIdRpContrib}{NoHtPasswordLogin}
Then if the user has logged in with OpenID in the past, they should be required to log in with OpenID in the future. In other word, if a user has logged in with OpenID in the past, but then tries to log in with their username and password (rather than their OpenID) it should give them an error message, assuming that the NoHtPasswordLogin flag is set.
