The process does

  • alter TWikiGuests,
  • adds to .htpasswd,
  • mails out,
  • but fails to create the users topic

Normally it shouldn't be TWikiGuest who is creating the new user but rather TWikiRegistrationAgent. You should be OK if you add TWikiRegistrationAgent to your ALLOWWEBCHANGE setting in Main.WebPreferences.
