Have we seen this bug before in Wysiwyg? Yes. Often.
Why does it come back again and again?

It is a configuration issue. the plugin relies on the 'HTML::Entities' module being installed, and on the path, and it cannot be installed on develop as there is no accessible C parser.

If this error happens consistently on a certain web or wiki (I'm not sure if it does tho), can the Wysiwyg plugin be disabled for that web? I know it can be disabled for an entire wiki.
Though I guess could result in people wondering why the Wysiwyg button dissappeared, so maybe a notice could be put in the Wysiwyg page (or in the error message) saying that Wysiwyg won't work on the develop web/wiki.

Yes. Just set COMPOSER.

I installed gcc and HTML::Entities, the INC error is gone, but the HTML edit area seems to contain more of the classic skin html than it should. CC?

It looks fine to me. It has it's own template, which is pretty crude. Really we need to switch to a better working model (I have a
TinyMCEPlugin that embeds a wysiwyg editor in the pattern skin, which is a lot neater).
